To obtain a transcript:

Include in your request you name as it was when you graduated/attended the School of Nursing, the date of graduation/attendance, social security number, and the complete address where you would like the transcript sent.  Transcripts for graduates of the Shepard-Gill School of Practical Nursing are also available through the MGH IHP.

Mail $5.00 fee payable to MGH IHP to:
